If your child is homeschooled and lives within the district you may still register for and participate in athletics at Farmington High School.

Most sports have strict registration deadlines because the tryout process starts on the first day of practice. Student-athletes are required to be fully registered and in attendance on the first day in order to be placed on a team. In some cases (illness, transfer students, extenuating circumstances), exceptions can be made for late registrations. This is at the discretion of the head coach and Activities Director.

YES! Parents can check online through Infinite Campus/Parent Portal to view the health history of their student by going to More > Health > Immunizations. The sports physical date is at the bottom of that page (most current data).

This is a great place to check if you submitted a sports physical and want to know if it has been entered in the system!

After a SPQE has been entered into your child's Infinite Campus portal, it takes 24 hours between the time it was entered to the time you can log in and register. If you are still unable to complete registration after waiting the 24-hour period, please contact the office at 651-252-2514. Please do not wait until the registration deadline to submit physical forms and expect to register that day.

Typically, we ask for a 24-hour grace period from the time a physical has been submitted to the time you log on to register. Sometimes the emails and faxes do not arrive to us at the time you expect, or the time the physician's office has told you. Since there can be a lag in time or communication, waiting 24 hours is usually necessary to make sure all of the information has been received and entered.

To be sure that your child's physical form is in the system, you can either log on to your Parent Portal through Infinite Campus and check the student's health screening profile - OR - you can attempt to register for the activity on RevTrak. If you receive an error message saying that your child's Student ID Number is incorrect, then it is likely that the physical has not been entered in the system. If you are able to answer all of the questions, add the activity to your cart, and complete registration and pay, then you can rest assured that the sports physical has been entered and all is well.

For the purpose of high school athletics, sports physicals are valid for 3 years (from the date of the original office visit). When the physical date has reached its expiration, your child will need to visit the doctor again and submit a new physical form.

Please note: Sports qualifying physical exams (SPQE) must be current through the end of a season. If the SPQE expires in the middle of a season, you will not be eligible to register until a more current SPQE is on file with the activities office.

The most common place to get a sports physical exam is the child's primary care physician or pediatrician's office. Any certified medical health professional can complete the physical exam and must sign and date the form.

There are select Walgreens stores and Minute Clinics that offer free sports physicals. You can search their websites to learn more information. Sport clinics like Twin Cities Orthopedics and Fairview Health also offer free sports physicals during the summer months and throughout the year. Pre-registration on their website is required.

No, you cannot complete online registration without a valid sports physical on file. All student-athletes must be examined by a certified health professional who has cleared them and deemed them medically eligible to participate in all school interscholastic activities. This is a requirement from the Minnesota State High School League. There is no waiver or exemption.
